Support Services For Patients And Loved Ones

Your Journey

Living with cancer or having a loved one be diagnosed with the disease can be overwhelming, discouraging and extremely stressful. At CCC, we understand the broad range of biopsychosocial needs of our patients and their families that come as a direct result of diagnosis.

As a patient organization our goal is to provide a space for patients and caregivers to learn about the disease and what they can expect throughout their experience. Since each person’s experience with the disease is unique, we offer one-on-one patient & caregiver phone or virtual consults to address individual questions and concerns. Whether it is a question about treatment options, management of treatment side effects, establishing clear communication with your medical team, resource support, or any other facet related to your colorectal cancer, we are here to guide you.

To provide national outreach to patients across Canada, CCC offers monthly virtual support groups via Zoom. The two-hour meeting includes a presentation on clinical or psychosocial CRC related topics followed by a Q & A/group discussion and then a check in with group members. Groups are moderated by our Patient Support Counselor and offer a safe space to engage in disease education and peer support.

With a focus on what is most important to the individual patient and their loved ones, CCC ensures that you are not alone in your journey. We invite you to reach out for and allow our team to help guide and support you through your cancer care.

My Colon Cancer Companion Patient Journal

My Colon Cancer Companion Patient Journal was inspired by conversations within the community of colorectal cancer patients, survivors, caregivers and health professionals. These conversations revealed that many patients required additional materials and assistance with non-clinical aspects of their experiences. In a perfect world, every patient would have a nurse navigator who they could access anytime to answer their questions or discuss medical or emotional matters that concern them — but they don’t. My Colon Cancer Companion was created to help fill this gap.
